Once you have completed all requirements for the thesis (including any public presentations, performances, oral exams, and so forth), submit the following thesis completion form. Refer to the Major and Thesis Requirements page for departmental requirements.

A decision request form will be sent to each thesis committee reader listed. Once your thesis has been reviewed by all three readers, the Honors Faculty Representative(s) and your Honors advisor(s) will be notified of an approval decision. UHP Admin will also be notified and will enter your information into your Honors APAS for degree clearance purposes.
